There will be a free immunization clinic at CCMS on Tuesday, December 17th, for those students who are not up-to-date with their vaccinations for the 19-20 school year. Please contact Anna Dunford (School Based Health Assistant), in our Main Office @ (503) 399-3701, if you have any questions. A parent consent form is required. Please see attachments for a copy of the consent form, district immunization requirements, and information about specific immunizations.

Immunization Exclusion Day is Wednesday, February 19, 2020. Your student must be up-to-date on their immunizations or they will be excluded from attending school.
